You are eligible to participate if:
- You have been diagnosed with Lyme disease that can be verified by a healthcare provider
- You are at least 5 years of age
- You are able to visit Danbury Hospital for an initial registration

Participation in the Western Connecticut Health Network Lyme Disease Registry is free. There is no cost to you or your insurance provider for participating in the Registry.
There is no compensation for your participation in the registry, including but not limited to compensation for participation, travel expenses, or treatment. This is a unique opportunity to contribute to a national real-time landmark research project. Your participation may help explain the complex aspects of this poorly understood disease. We very much appreciate your participation to help provide more information to our community.
What Can I Expect When I Register?
The Lyme Disease Registry participation process is simple. When you register, a research assistant will review the participation forms with you and ask for your signature. You will be asked a series of questions pertaining to your Lyme disease, including past diagnoses, your treatment history, and potential risk factors. About a tablespoon of blood will be drawn and used for various testing related to understanding more about Lyme disease. Every 6 months, the registry will contact you to complete a follow-up form. You do not need to return to the registry site to submit follow-up information.
Simple Process of the Lyme Disease Registry
- A Lyme Disease Registry research assistant will ask you a series of questions pertaining to your Lyme disease
- Your blood will be drawn at Danbury Hospital for tests related to learning more about Lyme disease
- With your consent, the research assistant will contact your physician to verify medical information pertaining to your Lyme diagnoses
- You will receive a follow-up survey every six months
Your Personal Health Information is Important to Us
We take patient privacy very seriously and want you to feel confident about your participation with the Lyme Disease Registry.
All information provided to the Lyme Disease Registry, including laboratory results and medical history, are coded using labels with a unique identification number. Only the investigator and designated site staff will have the key to link each identification number to the corresponding participant. The results of any analyses that are run are not linked to any given participant, and all research information is kept strictly confidential. The latest HIPAA Privacy Rule regulations are followed.
