Western Connecticut Home Care Receives $60,000 to Provide Telehealth to Patients

Western Connecticut Home Care (WCHC) was awarded $60,000 from the Connecticut Health and Educational Facilities Authority (CHEFA) to fund the Agency’s Telehealth Program. The Authority recently announced $1 million in grants to 28 of nonprofit institutions throughout the state that assist in providing food, healthcare and shelter.

Telehealth utilizes small electronic devices placed in a patient’s home which allows clinicians to monitor the patient’s vital signs from a remote location. Patients use the device and peripheral equipment to take measures of their blood pressure, pulse, oxygen level and weight. The information is sent electronically to be evaluated by a nurse. While Telehealth does not replace visits by the nurse, it does allow patients with chronic illness such as congestive heart failure (CHF) and chronic obstructive pulmonary disease (COPD) to be monitored every day. Daily monitoring allows the nurse to provide intervention as soon as negative changes to the patient’s health status is detected, helping to avoid symptoms becoming acute and requiring costly interventions such as a visit to the emergency room and/or hospitalization.
