Western Connecticut Home Care
Certified home health aides deliver personal care such as bathing or dressing. They can also shop for, and prepare, healthy meals that may be prescribed by a dietitian, as well as assist with laundry and household chores.
Home health aides from Western Connecticut Home Care undergo comprehensive training. Each home health aide has had a full background check and is carefully screened.
Our home health aides work under the guidance of your home healthcare nurse, physical therapist, or occupational therapist, and we take special care to match your needs with your aide's skills and abilities. Many of our aides are bilingual and we always do our best to match you with one who can address you in the language you speak most comfortably.
At Western Connecticut Home Care, we believe that using the most current technologies can help us provide you with the best care possible.
Our field staff, including nurses, therapists, aides, and other professionals, use technology for two primary purposes:
- Improve communication between us and your doctor; and
- Improve patient outcomes, to help you or your loved one through recovery or ongoing care
Wireless Access in the Field
Every one of our nurses, rehabilitation therapists, and social workers carries a laptop computer. They enter their notes from each visit directly into their computers. They can also access databases to answer questions in your home. Every computer is equipped with special security software to ensure the safety and privacy of your personal information.
Whether you take two medications or 20, there's always the chance they may interact in a negative way. When your nurse enters the medicines your doctor has prescribed into your chart, our medication database runs a check to see if there might be any interactions.
If your nurse recommends that you participate in our TeleHealth program, we will place an easy-to-use electronic device in your home, which allows you to check your vital signs and then transmit this data to a nurse coordinator.
If your results are outside what your home healthcare nurse says is normal for you, the coordinator will contact you or your nurse to figure out the cause. With TeleHealth, changes in your health are often caught early enough to keep you from returning to the hospital or visiting the emergency room.
If you have a wound, your nurse may ask to take photographs of it. These images will be viewed by registered nurse wound specialists, who will consult with your doctor and make recommendations.
Home Health Aide Telephony™
With homecare Telephony™, home health aide field staff can use a touch-tone telephone to enter visit information directly into a proprietary database. Patient information that is entered into Telephony is documented as visit notes.
Telephony™ gives home health aide staff the ability to enter supplies, time and travel, patient information, condition, and more. All patient information is paperless and saved to the Western Connecticut Home Care electronic medical record.
