Outpatient Diabetes Self-Management Education
Danbury Hospital offers an outpatient diabetes self-management education program, including specific diabetes-related nutrition counseling, plus other services to support your good health. We are experts in diabetes and you are the expert in your life; together, we can create a plan to meet your goals.
All program sessions meet at Danbury Hospital. Language interpretation services are available.
Recognized by the American Diabetes Association as meeting the National Standards for Diabetes Education, our program is taught by nurses and dietitians who are certified diabetes educators. Patients are offered individual office visits and comprehensive group classes.
Monthly Diabetes Self-Management Education classes consist of a series of four classes, each with a specific area of focus. Classes are interactive and designed to provide the information needed to minimize complications and maximize your quality of life. Morning and evening classes are held in Danbury, and afternoon classes are held in Southbury.
Topics we will cover include:
- Understanding diabetes
- Medication treatment
- Blood sugar monitoring
- Good habits to reduce complications
- Nutrition and meal planning
- Personalized goal setting
Insurance Benefits and Process
Medicare and most CT and NY health plans recognize the benefits of an ADA-recognized education program. We will be happy to discuss your insurance benefits and assist you with the referral process (a physician’s order is required).
